Support MetroHealth's Transitional Care Unit
David Wank is a training and quality improvement specialist at MetroHealth who also happens to be an ultramarathoner. An ultramarathon is any distance over the traditional 26.2 mile marathon. Typical ultrmarathons range from 50 kilometers to over 100 miles. On April 26, 2014 David will be competing in the Outrun 24 hour race. The race consists of a one mile trail loop at the scenic Lake Metroparks Chapin Forest Reservation in Kirtland, OH. The goal is to complete as many laps as possible in the designated 24 hour period. David is currently training hard, and hopes to complete at least 100 laps in support of MetroHealth's Transitional Care Unit.
The Transitional Care Unit is located on the Old Brooklyn Campus of the MetroHealth System and provides an outpatient service in a clinic setting to teach and assist patients to manage their chronic health conditions such as lung disease, diabetes, cellulitis and heart failure. The program offers a way for patients to be seen by health care professionals sooner and more frequently, when care is most needed, after an Emergency Department visit or inpatient admission. Unfortunately some of these patients don't have the financial resources needed to meet basic condition management needs. The good news is that you can help! With your donation these patients can bridge the financial gap that is keeping them from much needed medicine and medical supplies.
